1) She doesn't HAVE a dog.
or
2)She doesn't HAS a dog.

Option 1 is correct: She doesn't have a dog.
We use has only for 3rd Person singular in positive statements in the present simple tense. We use the base form of the verb have when making questions or negatives . You can see more information about forming the present tense on LearnEnglish Grammar.
